There is a particular kind of dread that hits when your table is ready, the wine list is gorgeous, and your body is already whispering, “Don’t do it.” I have lived that moment from both sides of the service well. As a beverage director, I watched guests push a glass away mid-sip, apologizing like their biology was being rude. As someone with a newly temperamental system, I get it now in my bones.

Let’s clear something up: people can react to wine for a lot of reasons, and not all of them are “an allergy to sulfites,” even though that is the phrase most of us have been handed. The industry has responded with low-histamine bottlings, organic and biodynamic farming claims, natural wine movements, and labels that say things like “no added sulfites.”

Some of it helps. Some of it is marketing shorthand. Most of it needs translation.

Quick safety note: If you ever have throat tightness, wheezing, fainting, swelling of lips or tongue, or rapidly spreading hives, treat it as urgent and medical. If you have known severe allergies, ask your clinician whether carrying epinephrine is appropriate. Wine is not worth guessing games.

What “wine intolerance” means

When guests tell me wine “doesn’t agree with them,” they usually mean one or more of these: headache, facial flushing, stuffy nose, itchy skin, hives, nausea, heartburn, or that wired, uncomfy feeling that makes sleep impossible.

Medically, the umbrella includes a few different mechanisms:

  • True allergy (immune response), which is rarer with wine but can happen with things like grape proteins, fining agents, or molds.
  • Intolerance (your body struggles to process certain compounds), sometimes tied to histamine, other biogenic amines, acetaldehyde handling, or alcohol itself.
  • Sensitivity or irritation, where additives, acidity, congeners, or (in sparkling wines) carbonation may be triggers.

For the rest of us living in the gray area, the goal is not to “win” against wine. It is to find the most comfortable path, including the option to opt out without feeling like you are ruining the vibe.

Histamine and biogenic amines

Histamine is a natural compound found in many fermented or aged foods and drinks: wine, beer, cured meats, aged cheese, soy sauce, kimchi. Some people seem to have a lower threshold for histamine load, whether due to baseline enzyme activity, gut inflammation, medication effects, or just plain bad luck in the genetics department.

Your body breaks histamine down largely via an enzyme called DAO (diamine oxidase). Alcohol may reduce DAO activity and can also make reactions feel louder, which is one reason wine can hit harder than, say, a small serving of sauerkraut. Mechanisms are still being studied, and responses vary a lot person to person.

Wine can contain histamine and other biogenic amines (like tyramine, putrescine, cadaverine). These tend to form when certain microbes are active during fermentation and aging. In broad strokes:

  • Red wines often test higher in histamine than whites and rosés, but it is not because “red equals histamine.” It is more that red winemaking conditions can sometimes favor amine formation, especially depending on microbial activity, pH, and malolactic fermentation choices.
  • Long macerations, warmer fermentations, and extended time before stabilization can increase risk if the wrong microbes get a foothold. They are not automatic culprits, but they are variables.
  • Cleaner, well-managed fermentations often reduce the risk of higher biogenic amines, even if the wine is still “natural” in spirit.

One important caveat: histamine is a plausible contributor for some drinkers, but it is not the universal explanation for “wine headaches.” Migraine biology, alcohol itself, sleep disruption, dehydration, tannins, and individual susceptibility can all be in play. Translation: if low-histamine wine does nothing for you, you are not imagining things.

Low-histamine wine

Low-histamine wine generally refers to wine that has been tested to contain histamine below a certain threshold. The exact cutoff depends on the certifier or lab protocol, and there is no single global standard that every winery must follow. Still, it is one of the few categories where you are at least talking about measurement, not just a vibe.

How it is typically made

  • Healthy fruit and strict hygiene to reduce unwanted microbial activity.
  • Careful yeast and bacteria management, including limiting strains associated with higher biogenic amine production.
  • Controlled malolactic fermentation (or avoiding it), since bacterial steps can influence amine levels.
  • Lab testing to verify histamine levels before release.

Who it tends to help

Low-histamine wines can be a game-changer for people who consistently react to certain wines with flushing, headaches, or congestion, particularly when those reactions also show up with other high-histamine foods.

Where it can disappoint

If your issue is primarily alcohol (vasodilation, sleep disruption, dehydration), tannins and phenolics, acidity, or residual sugar, low-histamine alone may not solve it. Also, “low histamine” does not automatically mean “no sulfites,” “organic,” or “additive-free.” Those are separate axes.

What I tell guests: low-histamine is a targeted tool, not a halo.

Organic wine

Organic wine is primarily a statement about how the grapes are grown, not a guarantee about how the wine will feel in your body the next day. Organic standards differ by country and certifying body, but the common thread is restricting synthetic pesticides, herbicides, and fertilizers in the vineyard.

Does organic mean fewer sulfites?

Not necessarily. Some regions allow organic wine with added sulfites up to certain limits. Others have stricter rules. Many organic producers use less sulfur because their farming and cellar practices are dialed in, but “organic” itself is not the same as “no added sulfites.”

Can organic help with intolerance?

Sometimes, depending on your trigger. If you suspect you are reacting to certain agricultural chemicals or you generally do better with fewer residues, organic can be part of a lower-noise approach. But if your symptoms line up with histamine overload, organic certification does not automatically correlate with low histamine levels.

Biodynamic wine

Biodynamic wine takes organic principles and adds a whole-farm ecosystem approach. In practice, biodynamics often means composting, biodiversity, limiting outside inputs, and following a biodynamic calendar for certain vineyard tasks. Many biodynamic estates also lean toward minimal intervention in the cellar, though that is not a requirement in the way many consumers assume it is.

Does biodynamic mean “cleaner” wine?

It can mean fewer additives and a more restrained hand in winemaking, depending on the producer. Biodynamic certifications tend to include tighter guidance than baseline organic standards. Still, biodynamic is not a direct proxy for low histamine or low sulfites. It is a proxy for a method and a mindset.

When it might matter

If you are trying to reduce exposure to certain vineyard inputs and you prefer producers with transparent, low-additive practices, biodynamic can be a helpful filter. Just keep your expectations realistic: you are choosing a farming and production ethos, not a medical accommodation.

Natural wine

Natural wine is more movement than definition. Generally, it signals: organically farmed fruit (not always certified), native or spontaneous fermentation, minimal additions, and little to no filtration. Some natural wines are pristine and stable. Others are funky, volatile, or referment in bottle. That variability is part of the romance, and also part of why sensitive drinkers can have mixed experiences.

Can it reduce reactions?

For some people, yes, especially if their trigger is a combination of higher sulfite additions and certain additives. But natural wine can also be higher in biogenic amines if microbial activity is less controlled, or if hygiene slips. That means a natural label can be either your best night out or your worst headache.

My industry take: natural wine is not automatically “gentler.” It is often more transparent in spirit, but it is not inherently more predictable.

No added sulfites

Sulfites are a family of sulfur-based compounds used widely in winemaking to prevent oxidation and microbial spoilage. They are also naturally produced during fermentation, which means truly “sulfite-free” wine is extremely rare in a strict sense.

What you will actually see most often is:

  • No added sulfites: the winery did not add sulfur dioxide (SO2), but the wine may still contain naturally occurring sulfites.
  • Low sulfites: sulfur was used, but in smaller amounts.
  • Contains sulfites: required in many places above certain thresholds.

Are sulfites the cause of wine headaches?

For most people, probably not. Sulfite sensitivity exists and is more commonly discussed in the context of asthma. When it happens, it tends to show up as respiratory symptoms rather than the classic “red wine headache.” Also worth noting: many foods contain sulfites at levels comparable to or higher than wine, like dried fruit, pickled foods, and some condiments.

That said, fewer sulfites can still matter for some drinkers, and it can matter for the wine’s overall additive profile. Just do not let sulfites take the blame for everything when histamine, alcohol, hydration, and sleep are all waiting in the wings.

What to choose

Think of these as different filters for different problems.

  • If you get flushing, congestion, or headaches, especially from reds: start with low-histamine options if you can find them, then test styles (lighter reds, young wines, certain regions) and serving size. If flushing is intense, it is also worth looking into alcohol flush response and acetaldehyde processing (ALDH2 variants), which can make even small amounts feel like a siren.
  • If you want fewer synthetic vineyard inputs: choose organic or biodynamic.
  • If you suspect you react to higher sulfur additions or you want a minimal-additions approach: explore no added sulfites wines, but be mindful they can be less stable once opened.
  • If you want minimal intervention overall: try natural wines, but pick reputable importers, shops, or wine directors who taste with stability in mind.

In an ideal world for sensitive drinkers, you find a producer who is thoughtful across the board: healthy farming, clean cellar work, modest sulfur, and lab testing where appropriate. In the real world, you are often balancing trade-offs.

Ordering without making it weird

As someone who has written menus and also needed accommodations, I can promise: clarity is kinder than suffering silently.

What to say

  • Simple and useful: “I react to some wines. Do you have a low-histamine option, or something lower alcohol, lighter-bodied, and not heavily oaked?”
  • If sulfites are your concern: “Do you have anything with no added sulfites or a producer known for low sulfur?”
  • If you are not sure: “I tend to get headaches with big reds. Could you steer me toward something lighter, lower alcohol, and fresher in style?”

What usually works best

  • Lower alcohol by volume (often 11 to 12.5 percent) can be gentler.
  • White, rosé, or chillable red styles can reduce histamine and tannin load for many people.
  • By-the-glass lists from thoughtful programs are often curated for balance and drinkability, not just power.

If you want a few practical starting points to ask for, these are often easier for sensitive drinkers, with the usual caveat that producer and ABV matter: a crisp, dry Muscadet-style white, a dry Grüner Veltliner, or a light, low-oak red served with a chill (think Beaujolais-inspired styles). Start with a small pour and see how your body votes.

And if you are in a group setting, it is okay to order a non-alcoholic pairing, sparkling water, or an NA aperitif. Hospitality is supposed to meet you where you are. If a place makes you feel awkward about that, they are missing the point.

Other triggers people blame on “wine”

  • Alcohol and sleep: even one or two glasses can fragment sleep and amplify next-day headaches.
  • Hydration: wine is not uniquely dehydrating compared with other alcohol, but alcohol can increase fluid loss and it is easy to forget water when you are having fun.
  • Tannins and other phenolics: can be a migraine trigger for some people.
  • Acidity: can worsen reflux or sinus feelings.
  • Residual sugar and richness: not a diagnosis, just a pattern. Some people report feeling worse with sweeter or richer wines, especially when it stacks with higher alcohol and poor sleep.
  • Fining agents: rare, but some wines use egg white, milk casein, or fish bladder derivatives (isinglass). Disclosure rules vary by country, and residues are usually low, but if you have severe food allergies, ask directly.

How to experiment safely

If you are trying to pinpoint what sets you off, treat it like a calm little tasting project, not a Friday dare.

  • Change one variable at a time: try the same producer across white vs red, or low-histamine vs regular, rather than three new bottles at once.
  • Track the basics: grape, region, alcohol percentage, how much you drank, what you ate, sleep quality, and symptoms timing.
  • Start with smaller pours: a 3 to 4 oz pour can tell you a lot.
  • Eat and hydrate: boring advice, but it is the difference between signal and noise.

When to stop experimenting: if reactions are frequent, escalating, involve breathing, swelling, or severe hives, or you are starting to dread eating and drinking in public, bring the pattern to a clinician. An allergist or gastroenterologist can help you sort out what is plausible, what is risky, and what is just loud folklore.

Is red wine always higher in histamines?

Often, yes, but not always. Many reds trend higher, but individual producers and processes matter. A carefully managed red can be easier than a chaotic one.

Are organic wines better for headaches?

They can be, but organic is not a direct promise about histamines or sulfites. If organic wines help you, that is valuable information about your personal triggers, but it is not universal.

Does biodynamic mean no sulfites?

No. Some biodynamic producers use sulfur, some use very little, and some use none added. You still need to look at producer notes or ask the shop or sommelier.

If I am sensitive, what is the safest style to try?

Many sensitive drinkers do well starting with a dry white or rosé with lower alcohol, or a very light red served slightly chilled. If histamine seems to be the issue, seek out wines marketed and verified as low-histamine when possible.

Wine is supposed to be pleasure, not a punishment. The good news is that the industry is getting better at speaking the language of sensitivity, even if the labels still lag behind the lived experience. Ask questions. Choose producers who do the work. And if your body says no, listen. There are plenty of ways to belong at the table.
